All HYPA Case Management services are delivered by staff with tertiary education in Social Work or a related discipline. Staff receive additional training in HYPA’s Case Management Model which uses a variety of therapeutic interventions such as Motivational Interviewing, Narrative Therapy or Cognitive Behavioural Therapy and Positive Psychology (PERMA+). Our Case Management services are augmented by access to a range of HYPA specialist services and broader SYC programs and support structures.

Specialist Case Management

HYPA has developed intensive Case Management services for young people requiring a higher level of support. We tailor support to those experiencing homelessness or housing insecurity, those who are at risk of becoming or are engaged with the youth justice system and those experiencing high levels of anxiety, depression or mental health challenges. As a result these students are usually significantly disengaged within one or all of the three dimensions referred to within the DECD Student Engagement Matrix, ie Wellbeing, Relationships and Involvement in Learning.

General Case Management

HYPA’s evidence-based FLO Case Management Model is a collaborative process of assessment, planning, facilitation and advocacy to meet the living, learning and transition requirements of each individual FLO student. Our service is adaptable depending on the needs of each young person. We provide a full outreach case management service including meeting at community centres, the student’s home, at one of our HYPA offices school as appropriate and negotiated with the student, family and/or school.
